Within the scope of our Home Healthcare Services, medical examinations, diagnostic testing, treatment, and nursing care are organized in the home setting. When necessary, the process is managed through a multidisciplinary approach with consultations from relevant medical specialists.

Home-Based Pressure Injury Management

What Is a Pressure Injury?

A pressure injury is localized damage to the skin and/or underlying tissue, usually occurring over bony prominences such as the sacrum, hips, or heels. It results from prolonged pressure alone or in combination with friction and shear forces. If pressure is not relieved, tissue necrosis may develop over time.

  • Bedridden patients
  • Advanced age
  • Chronic diseases (diabetes, heart failure, neurological disorders)
  • Malnutrition
  • Urinary or fecal incontinence
  • Excess moisture and improper positioning

Home-Based Pressure Injury Risk Assessment

During the initial evaluation, the following are assessed:

  • Activity level
  • Nutritional status
  • Moisture exposure
  • Friction and shear risk
  • Medical device-related pressure risk

An individualized care plan is implemented for high-risk patients.

Home-Based Treatment and Follow-Up of Pressure Injuries

Skin Care and Moisture Management

  • Daily moisturizing of dry skin
  • Avoiding unnecessary massage
  • Reducing moisture exposure
  • Using barrier creams when indicated

Pressure Reduction

  • Repositioning bedridden patients at least every 2 hours
  • Hourly repositioning for wheelchair-dependent patients
  • Preferring 30° lateral positioning
  • Using pressure-redistributing mattresses and support surfaces

Nutritional Assessment

  • Evaluation of protein and hemoglobin levels when indicated
  • Individualized nutritional planning
  • Use of supplements when necessary

Wound Care and Continuous Monitoring

  • Assessment of wound size and stage
  • Monitoring for infection and exudate
